TeleHealth and Pharmacy
- Talk to a doctor any time day or night, on the weekend or when traveling and away from home. No inconvenience or hassle of traveling to the doctor’s office, urgent care or ER and waiting to be seen.
- If a physician recommends medication as part of the treatment plan, the prescription will be digitally sent to the local pharmacy of choice.
- Our membership card will allow you to fill your prescription at any of 64,000 pharmacies nationwide.
- The most utilized acute medications in our formulary can be purchased up to a 21-Day supply for a $5.00 payment.
- The top 75 most utilized chronic medications in our formulary can be purchased in a 90-Day supply mailed conveniently to your home.
Dental
The following is a list of the types of charges covered by the plan*:
- Checkups & Routine Cleaning
- Bitewing X-Ray
- Fillings
- Endodontics (root canal & pupal therapy)
- Periodontics (treatment of gums)
*Percent of charges the plan pays & waiting period of continuous enrollment before plan pays varies between types of charges covered by the plan.
Accident Coverage
Pays for employee benefits for injuries received in accidents on or off the job. These payments can help offset expenses incurred such as time off work, medical and non-medical expenses resulting from an accident.
- $1500 Hospital Admission Payment
- $300 per day Hospital Confinement
- $100 Health Screening Rider
- $125 – $7500 for Fractures or Dislocations
- $50,000 Accidental Death Benefit
- $25,000 Catastrophic Accident Benefit
Indemnity Health Plan
Our value-based offering that covers all the bases for preventive and wellness services, yet also includes enhanced indemnity benefits for inpatient, outpatient, primary care and prescription drugs.
Key Plan Features:
- Primary care and specialist office visit benefits included
- Limited/indemnity benefits include outpatient lab, outpatient surgery and inpatient hospitalization
- 24/7/365 telehealth consultations at $0 copay and patient cost
- Generic prescription and other prescription card options available
- No medical underwriting/No pre-existing condition limitations/No waiting periods
